Connected Self Psychology Practice

Connected Self Psychology is committed to protecting your privacy and maintaining the confidentiality of your personal and health information. We recognise the importance of privacy and comply with the Australian Privacy Principles in the Privacy Act 1988 (Cth) and Health Privacy Principles under the Health Records Act 2001 (Vic).

Scope of Personal and Health Data Collection

We collect information necessary for psychological services, including: name, DOB, address, contact details, emergency contacts, Medicare/insurance info, referral data, medical and family history, session disclosures, and billing records. Information is usually gathered from you directly or from relevant third parties with your consent.

Purpose of Information Collection

Your data is collected to provide psychological assessment and treatment, maintain accurate clinical records, communicate regarding appointments, and process payments or Medicare claims. It also allows us to liaise with other healthcare professionals (with your consent) and meet our legal, ethical, and professional clinical obligations.

Information Storage and Security

Records are stored using password-protected electronic systems or secure physical storage to prevent misuse or unauthorised access. In Victoria, health records are generally retained for seven years from the last contact, or until a client reaches 25 years of age if they were under 18 at the time of service provision.

Disclosure of Your Information

Data remains confidential unless you provide consent, disclosure is required by law, or there is a serious risk to safety. This includes legal subpoenas, mandatory reporting for child protection, and sharing with healthcare providers involved in your treatment where clinically appropriate and approved by you as the client.

Access, Site Use and Contact Details

You may request access to or correction of your information by emailing info@connectedselfpsychology.com.au. Our website uses cookies to improve experience. If you have unresolved privacy concerns, you may contact the Office of the Australian Information Commissioner (1300 363 992) or the Health Complaints Commissioner (1300 582 113).
